Onboarding a new contractor

This form should be completed by the program, not by the contractor.

Please do your best to submit this form at least two weeks in advance of the start date to provide sufficient time for review and gathering signatures. Thank you in advance.

Using either the consultant’s or vendor’s agreement, or Goodnation’s template (found in the shared Google Drive), complete accordingly and submit below without signatures so that Goodnation can review and manage gathering signatures. Note that all agreements should be between your vendor and Goodnation Foundation or Goodnation Action (depending on who your sponsor is).
