To make adjustments to a 1099 agreement, such as scope of work, pay, or term dates, please submit the below form

Use Goodnation’s addendum template (found in your Ribbon account), complete accordingly and submit below without signatures so that Goodnation can review and manage gathering signatures. Note that all agreements should be between your vendor and Goodnation Foundation or Goodnation Action (depending on who your sponsor is).
